Once you complete your registration, you will receive all these details by email. However, you should know that all participants of Ultra Paine® must complete the following forms by no later than 20 de septiembre de 2025:

1. Medical Record Form provided by the organization. This form can be accessed here.
2. Medical Certificate Form indicating that you are in good health to complete the distance in which you have chosen to participate. This form can be downloaded here.
   • If you have a Gmail account, you can upload the forms by selecting “Upload Files”.
   • If you don’t have a Gmail account, you must send the forms to the following email addresss: info@ultrapaine.com
3. Liability Waiver & Release Form. This will be sent via email, as you must use Adobe Acrobat Sign to digitally sign the form.
